What you'll do
- Identify and prioritize patients’ needs and maintain continuity of care.
- Oversee patient care plans with healthcare advocates, including support for social determinants of health.
- Learn and use Solace’s systems, tools, technology, and partner workflows.
What we're looking for
- Reside in Missouri and hold an active DEA license and HAP.
- Board certification in family medicine, internal medicine, or a relevant specialty.
- Strong clinical, organizational, communication, and documentation skills.
- Empathy and comfort working with elderly patients and people experiencing social determinants of health.
- Ability to learn new software and adapt to a rapidly changing environment.
